[Simh] DZ and VH device oddities under RSTS/E 10.1

I'm not too familiar with V10 of RSTS/E, but have encountered a similar 
problem with V9. I believe it's even in the SIMH arcvhives somewhere.

Modem support has to be built in when generating the RSTS/E monitor SIL. 
If you don't answer "YES" to the relevant question in the SYSGEN 
dialogue, RSTS will never know that it should pick up modem signals.
