[Simh] How to generate a RSTS V4 system? (fwd) (CrossPost from Rsts.org to SimH)

I received the following email this morning from a user and replied based
on my current information.  Thought I would post it here since SimH was
mentioned.  If anyone (other than myself) "IS" running a pre V7.0 RSTS on
SimH, I'm sure that Al, Tim and I would like to know.  Thanks.
