[Simh] SimH migration from XP to ME

XP uses a multimedia time resolution of 1msec, while ME and other 16b systems use a 55msec resolution.  This will screw up clock calibration big time.

If you build SimH natively on ME, you may get a version of the library that works correctly.  Otherwise, it will take source changes, eliminating the idle/throttling functions and reverting to the previous way of keeping time, to make SimH work on ME/98/95.
