[Simh] simh and RSTS/E - problems with getting additional terminal interfaces to work

I didn't say it clearly, but... around V9, the terminal driver was reorganized to have "port drivers" which are separately loadable. And those are always included with the monitor. So in those versions, there is no longer a terminal configuration section in the Sysgen dialog, and instead you get the terminals that are in the hardware. To get more, just add some controllers and restart.
